Failure to submit this form prior to the date listed on your summons will result in a personal summons delivered by the Logan County Sheriff's Office to your home. Your juror number for logging in is located at the top of your summons. 

When to report for jury duty: Call the Jury Commission Office at (217)732-4414 after 5:00 PM the business day prior to your service date. The recorded message will tell you what time you are to report on your assigned day and/or give further instructions. This message will be available on weekends prior to jury weeks. Alternatively, you may choose to download the Logan County Circuit Clerk mobile app to view Juror Instructions or opt-in to the Logan County Juror Text Notification System when you complete your questionnaire (included with your summons).
